	Goal: 3.This is a simple course evaluation system. The first page is a form that includes two text boxes for the student's name and the course number (such as CS 212, CS 480, MAT 171 etc). The second page lists two made-up questions for evaluating the course and three radio buttons showing the values of 'strong agree', 'agree', and 'not agree'. The third page displays all the information passed down from the last two pages, including student's name, course number, and results for the two questions. Another option for the course number is to use the selection list that is not required.
